By selecting the “I Accept” button, you are signing this Agreement electronically. You agree your electronic signature is the legal equivalent of your manual/handwritten signature on this Agreement. By selecting “I Accept” using any device, means or action, you consent to the legally binding terms and conditions of this Agreement. You further agree that your signature on this document (hereafter referred to as your “E-Signature”) is as valid as if you signed the document in writing. You also agree that no certification authority or other third party verification is necessary to validate your E-Signature, and that the lack of such certification or third party verification will not in any way affect the enforceability of your E-Signature or any resulting agreement between you and CTF inc.

I understand and certify by signing below that martial arts camps can be an extremely rigorous physical activity, and while the camp directors do as much as possible to provide safe training facilities, no such camp can guarantee the absolute safety of participants. I hereby agree not to hold the CTF, any instructor nor fellow camper responsible for my well being or any injuries that may occur during camp. While waiving responsibilities, I also warrant that I have personal medical insurance to cover me while participating in camp activities.

WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19 ASSUMPTION OF RISK

In consideration of being allowed to participate on behalf of (insert name of sports organization) athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I certify that I DO NOT meet any of the following characteristics:

a) Have had a fever of 100.4°F or greater in the last 2 days;

b) Have a cough, difficulty breathing, sore throat, or loss of taste or smell;

c) Had contact with a person known to be infected with COVID-19 in the previous 14 days.

5. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Choong Sil Taekwondo Federation, Inc., their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

IMPORTANT: THIS STATEMENT MUST BE SIGNED FOR PARTICIPATION IN CTF BLACK BELT CAMP


By my acceptance above, I hereby give my permission for my child to participate in all activities at the CTF camp.
In the event that he/she is injured while under supervision of CTF staff and requires the attention of a doctor, I give permission to the physician selected by the staff to order x-rays, routine tests, and treatment for my child. Also, in the event that I cannot be reached in an emergency, I hereby give permission to the physician selected by the staff to hospitalize, secure proper treatment for, and to order injection and/or anesthesia and/or surgery for my
child as named above. In the event it becomes necessary for the CTF staff to give consent for use, we agree to hold such person and the CTF free and harmless of any claims,demands, or suits for damages arising from the giving of such consent so long as the treatment is administered by or under the supervision of a licensed physician.
